Job Description: The Clinical Data Abstractor II will be responsible for reviewing, abstracting, and analyzing clinical data from various sources, ensuring accuracy and completeness in accordance with regulatory standards and organizational protocols. Supports quality improvement, clinical programs, and patient care initiatives through chart abstraction, data collection, and other administrative tasks. Key Responsibilities and Tasks: Compare medical chart notes with HEDIS/PQA/IHA measure specifications and guidelines to determine if documentation meets requirements for quality measures. Extract and input clinical data from electronic health records, patient charts, and other relevant sources into quality reporting and analytics platform for measure compliance. Conduct supplemental data reviews according to quality program priorities. Ensure data accuracy,
