As an Intake Care Representative, you will be a trusted first point of contact for patients, providers, hospitals, and referral partners. This role involves handling inbound calls from patients, helping them navigate the referral and intake process while ensuring accurate information is collected and care can begin without delay. Hours: Monday-Friday 9am-6pm EST, rotating shift (some weekend/holiday coverage 8am-4:30pm EST) Responsibilities Serve as the primary point of contact for patients, responding to calls and providing timely, professional support throughout the intake process. Help patients understand next steps, gather required information, and connect them with the appropriate care teams to support a positive patient experience. Handle inbound and outbound communication with patients, providers, hospitals, physician offices, and healthcare facilities regarding referrals and care coordination. Support timely patient onboarding by gathering, reviewing, and entering referral information accurately into company systems. Receive and triage referral packets, medical orders, and supporting documentation to the appropriate teams for processing. Coordinate home health services based on patient needs, geographic location, and insurance requirements. Verify patient treatment timelines, including first-dose administration and home infusion scheduling needs. Upload and maintain accurate patient records and documentation within the medical record system. Partner with Intake Coordinators and nurses to ensure referrals move efficiently through the intake process. Communicate referral updates and pendin
