About the Role: The Regional Practice Manager, Patient Care Coordination (RPM) owns the day-to-day performance of the Patient Care Coordination (PCC) team within their assigned region. Working through a direct-report Lead, the RPM ensures the team consistently meets key performance indicators, delivers high-quality patient support, and runs efficient, reliable workflows.
